Part 1
This application-based program is designed to help pharmacists provide medication therapy management to seniors. Guidance on
establishing or expanding MTM services and assistance in identifying resources specific to geriatric Pharmacotherapy will be
provided. This web-based program will use active learning techniques, case-based studies and lecture. Cost: $50.00 5.0 Contact

This program is part I of a 21-hour certificate program. Completion of all three components is required to receive a
certificate as Trainer in Medication Therapy management for Older Adults.
Part 2
This one-day application-based workshop will provide pharmacists with additional training in medication therapy
management for older adults as well as provide training in providing quality educational experiences in geriatrics for pharmacy
students in the practice setting. Using case studies, participants will work in teams to identify drug related problems and
formulate rational interventions. Participants will be able to describe the rationale for the correct therapeutic decisions made in
the case-studies. Cost: $65.00 8.0 Contact Hours or 0.8 CEUs
In order to register for this workshop, participants must have successfully completed the web-based component of this
certificate program.
Part 3
This application-based program is a mentored implementation of an APPE in the participant’s practice site. This program will focus on
the community practice setting. An internet site will be used to facilitate mentoring and communication among faculty and trainees.

In order to register for this workshop, participants must have successfully completed the web-based and workshop
components of this certificate program.
